The Ermital release form is similar. It is available in the form of microtablets coated with a film coating that is resistant to the action of gastric juice. Microtablets are contained in a common gelatin capsule.

What is the difference between Creon and Hermital in dosage? Rationing of drugs of this type is carried out not on the dry weight of the substance, but on the activity of lipase (a unique pancreatic enzyme) and is expressed in units of activity.

Creon on our pharmacological market is represented by the brands Creon 10000 and Creon 25000 with the corresponding dosages of lipase:

The active substance pancreatin, isolated from pancreatic tissue (PG) of pigs, includes pancreatic enzymes (lipase, amylase, protease) necessary for the digestion of fats, carbohydrates and proteins. Creon is available in three dosages: 10,000, 25,000 and 40,000 units (U).

Creon 10,000 contains 150 mg of pancreatin in one capsule with enzyme activity:

  • lipase – 10,000 units;
  • amylase – 8,000 units;
  • protease – 600 units.

The cost of a drug with a dosage of lipase 10,000 varies from 280 to 320 rubles per package of 20 capsules.

Dosage form of Creon

Proteases are needed to digest proteins. There are several types of them, they are produced not only in the pancreas, but also in the stomach, so deficiency of amylase and proteases is not as critical as the absence of lipase.

When and how are enzymes prescribed?

Enzyme replacement therapy is needed when postprandial (after eating) pancreatic enzyme secretion falls below 10% of normal levels. Due to impaired absorption of nutrients, the following is observed:

  • weight loss,
  • flatulence, diarrhea,
  • steatorrhea,
  • anemia,
  • hypovitaminosis.

Enzyme preparations should be prescribed by a doctor after examination and conclusion on the advisability of enzyme therapy. Mandatory practice is to select the dose of medication in accordance with the severity of enzyme deficiency and the composition of the diet.

For adult patients, the recommended dose is 50,000–75,000 U of lipase per main meal and 25,000 U for a snack. Patients are instructed that the dose can be increased according to the consumption of fatty foods, but dietary requirements must still be followed. Symptoms of steatorrhea and weight loss indicate an insufficient dose of the enzyme and the need to adjust it.

How effective are enzyme products?

Over the many years of clinical use of Creon, more than 20 placebo-controlled randomized studies have been conducted. A retrospective analysis showed that in various forms of exocrine pancreatic insufficiency (chronic pancreatitis, gland resection, cystic fibrosis), the use of Creon reduced steatorrhea in patients by 25–30% compared with the placebo group.

The rate of protein absorption in patients taking the drug for several months was 97%, while in the placebo group it was only 24%.

At the same time, side effects in the form of gastrointestinal symptoms were observed in the group taking Creon no more often than in the placebo group. Interestingly, when patients were prescribed drugs to reduce acidity (proton pump inhibitors, histamine H2 receptor blockers) in combination with enzymes, the incidence of complications increased.

Cost of courses for adults and children

If we compare Creon and Ermital, the price of drugs with the same activity differs by almost 2 times. The price of Creon 25,000 (20 capsules) ranges from 600 to 650 rubles. Ermital of the same activity and in the same quantity costs from 340 to 380 rubles.
